Calif. Group Launches Patient-Centered Medical Home Initiative
CPCA piloted the patient-centered care initiative for more than one year with 150 health care providers, according to a CPCA release (Robeznieks, Modern Physician, 10/11).
Details of the Initiative
Carmela Castellano-Garcia, CPCA CEO, said, "The initiative builds on the team-based approach to care that health centers have used as their model for many years and leverages their existing strengths to build a sustainable standard for health home activities and services."
The goal of the initiative is for all of the association's 900 members -- most of which are community clinics -- to be recognized by the National Committee for Quality Assurance for providing timely, consistent and coordinated care (Payers & Providers, 10/11).
Tools Available to CPCA Members
The initiative will provide CPCA members with access to:
- An online portal developed to help members follow the NCQA implementation roadmap;
- Customized quality-improvement coaching;
- Data management and analytics tools; and
- Online training (Modern Physician, 10/11).
